Product Description:

Elotuzumab works differently from most therapeutic antibodies in that it barely kills anything by itself. Its target, SLAMF7, is expressed on myeloma cells but also on natural killer cells, and binding it has opposite effects on the two: on the myeloma cell it flags the cell for destruction, while on the NK cell it acts as an activating signal. The result is a directed immune attack rather than direct cytotoxicity, which is why it is licensed only in combination.

It is used for multiple myeloma in patients who have received prior therapy, combined either with lenalidomide and dexamethasone or with pomalidomide and dexamethasone. Adding it to those backbones extends progression-free survival without adding much toxicity of its own. Infusion reactions are the main acute issue and require premedication with an antihistamine, paracetamol and a corticosteroid, with a graded infusion rate. Lymphopenia, fatigue, diarrhoea and increased infection risk occur, and it interferes with serum protein electrophoresis.
