Product Description:

Cetuximab is a chimeric mouse-human monoclonal antibody against the epidermal growth factor receptor. It binds the receptor's external domain with far greater affinity than the natural ligand, preventing dimerisation and shutting down the downstream signal. The critical point is that this only helps if the pathway below the receptor is intact: tumours carrying RAS mutations signal independently of EGFR, so the drug is useless in them and testing is mandatory before use.

It is given by intravenous infusion for RAS wild-type metastatic colorectal cancer and for squamous cell carcinoma of the head and neck, where it is combined with radiotherapy or platinum chemotherapy. The characteristic toxicity is an acneiform rash, whose severity correlates with response. Severe infusion reactions occur, most often with the first dose and at higher rates in regions where anti-galactose-alpha-1,3-galactose antibodies are common. Hypomagnesaemia requires monitoring.
