Product Description:

Vismodegib blocks Smoothened, a receptor in the Hedgehog signalling pathway. That pathway governs tissue patterning during embryonic development and is largely silent afterwards, but almost every basal cell carcinoma reactivates it through loss of the PTCH1 brake. Blocking Smoothened switches the pathway off and can shrink tumours that surgery and radiotherapy cannot address.

It is used for metastatic basal cell carcinoma and for locally advanced disease that has recurred after surgery or in patients unsuitable for surgery or radiation — typically extensive facial tumours where resection would be disfiguring. It is also used in Gorlin syndrome, where patients develop very large numbers of lesions. The adverse effects trace directly to inhibiting a developmental pathway: muscle spasms, loss of taste, alopecia and weight loss, together troublesome enough that many patients interrupt treatment. It is severely teratogenic.
