Cisplatin is used in the treatment of ovarian, cervical and testicular cancer.

Cisplatin is a platinum coordination complex, and the drug that made solid-tumour chemotherapy curative rather than merely palliative. Once inside the cell, chloride ligands are displaced by water and the resulting reactive species binds DNA, forming intrastrand cross-links that distort the helix and trigger apoptosis. Its activity spectrum is broad, and it remains the backbone of treatment for testicular cancer, where cure rates exceed ninety per cent.

It is used for testicular, ovarian, cervical, bladder, head and neck, oesophageal and non-small cell lung cancers, given by intravenous infusion with mandatory pre- and post-hydration. Three toxicities define its use: nephrotoxicity, which hydration and magnesium repletion mitigate; irreversible high-frequency hearing loss, particularly in children; and severe emesis requiring three-drug antiemetic prophylaxis. Peripheral neuropathy accumulates with dose.

Sourcing notes. Off patent and widely available as a 1 mg/mL solution, so the vials quoted correspond to 10 mL and 50 mL. Supply has nonetheless been fragile, with repeated shortages traced to a handful of sterile injectable plants. Cytotoxic handling, protection from light, and no refrigeration — cisplatin precipitates if chilled.
