Cabazitaxel is used in the treatment of advanced prostate cancer. It is used to treat cancer of the prostate gland that has not responded to treatment with other anticancer medicines. It may be used alone or in combination with steroid medicines.

Product Description:

Cabazitaxel is a semi-synthetic taxane that stabilises microtubules, preventing the dynamic assembly and disassembly that mitosis requires. Its design point is resistance: it has poor affinity for P-glycoprotein, the efflux pump that tumours use to expel docetaxel and paclitaxel, so it retains activity in tumours that have become taxane-resistant.

It is used with prednisone for metastatic castration-resistant prostate cancer previously treated with a docetaxel-containing regimen, where it demonstrated a survival benefit — the first agent to do so in that setting. Severe neutropenia is the dominant toxicity and the reason for the boxed warning; primary prophylaxis with growth factor is recommended in patients at higher risk, and deaths from neutropenic sepsis occurred in trials. Hypersensitivity reactions require premedication with antihistamine, corticosteroid and H2 blocker. Diarrhoea can be severe. It is contraindicated in significant hepatic impairment.

Sourcing notes. Patent position varies by market and generics exist in some. It is supplied as a concentrate requiring two-step dilution — a preparation sequence that has caused dosing errors, so pharmacy familiarity matters. Cytotoxic containment and closed-system transfer apply.
