Product Description:

Carboplatin is a platinum compound that forms cross-links between DNA strands, blocking replication and triggering cell death. It was developed to keep cisplatin's activity while removing its worst toxicities, and it largely succeeded: the cyclobutane dicarboxylate ligand makes it more stable and less reactive, so it causes far less kidney damage, nausea and hearing loss — at the cost of considerably more myelosuppression, particularly thrombocytopenia.

It is used across a wide range of tumours — ovarian, lung, head and neck, bladder, endometrial and germ cell — usually in combination. Its dosing is distinctive: rather than body surface area, it is dosed to a target area under the curve using the Calvert formula, which incorporates renal function, because clearance is almost entirely renal. Getting that calculation wrong is a recognised cause of serious overdose. Hypersensitivity reactions increase with repeated cycles, particularly beyond six.

Sourcing notes. Off patent, inexpensive, on essential medicines lists, and one of the highest-volume cytotoxics in the world. A cytotoxic needing containment at manufacture and closed-system transfer in pharmacy. Vial sizes and concentration should be stated, since dosing is calculated rather than fixed.
