Etoposide is used to treat different cancers of blood, lung, and testicles. It is also sometimes used to treat some other kinds of cancer in both men and women. It can be used on its own, or sometimes given together with certain other medicines as part of combination chemotherapy.

Etoposide injection delivers the same topoisomerase II poison as the oral capsule, but with complete and predictable exposure, which is why it is the form used in curative-intent regimens where dose matters precisely. By stabilising the enzyme-DNA complex after the strand has been cut, it converts a routine repair intermediate into a lethal double-strand break, and the effect is strongly schedule-dependent — divided daily dosing over three to five days outperforms a single large dose.

It is a backbone of small cell lung cancer treatment with a platinum agent, of BEP and EP regimens that cure metastatic testicular cancer, and of lymphoma, sarcoma and conditioning protocols before stem cell transplant. Myelosuppression is dose-limiting. Hypotension occurs if the infusion runs too fast, so it is given over at least thirty to sixty minutes. The concentrate must be diluted adequately, since it precipitates above a threshold concentration.

Sourcing notes. Off patent, widely multi-source, and one of the lowest-cost agents in oncology, though sterile injectable capacity has proved fragile. Note that etoposide phosphate is a different, more soluble product and is not quoted against this one. Cytotoxic containment; room temperature, protect from light.
