Cyclophosphamide is used in the treatment of different kinds of cancers. It is also used to treat kidney disease known as nephrotic syndrome in children after other treatments have not worked. It may be also used to treat other conditions, as determined by the doctor.

Product Description:

Cyclophosphamide is a prodrug — inert until the liver converts it, by cytochrome P450, into phosphoramide mustard and acrolein. The first cross-links DNA and kills dividing cells; the second is a urothelial irritant and the reason the drug is associated with haemorrhagic cystitis. At high doses it is a cytotoxic; at low doses it suppresses lymphocyte function, which gives it a second life entirely outside cancer.

In oncology it is used for lymphomas, leukaemias, breast cancer, sarcomas and neuroblastoma. In non-malignant disease it treats lupus nephritis, vasculitis, scleroderma and, as the tablet strength here suggests, childhood nephrotic syndrome resistant to steroids. Besides myelosuppression and cystitis, the significant risks are infertility, which is dose-related and often permanent, and secondary malignancy including bladder cancer years later. Adequate fluid intake, and mesna at higher doses, reduce urothelial damage.

Tablets require cytotoxic handling despite the oral route, and must be stored below 25 degrees — the substance melts and degrades above that, which is a real constraint for warm-climate distribution.
