Deutetrabenazine is a CNS-active medication used to treat involuntary movements (chorea) in Huntington’s disease and tardive dyskinesia. Product Description:
Deutetrabenazine inhibits vesicular monoamine transporter 2, depleting dopamine from presynaptic vesicles and reducing the excess dopaminergic signalling that produces involuntary movement. Its distinguishing feature is chemical: six hydrogen atoms are replaced by deuterium, which strengthens the carbon-hydrogen bonds that metabolising enzymes must break. Clearance slows, plasma levels are steadier, and the same effect can be achieved at roughly half the dose of tetrabenazine with less peak-related sedation and depression.
It is indicated for chorea associated with Huntington's disease and for tardive dyskinesia, taken twice daily with food and titrated slowly. The class carries a boxed warning for depression and suicidality in Huntington's patients, who are already at elevated risk, and treatment requires active monitoring of mood. Parkinsonism, somnolence, akathisia and QT prolongation occur; dose limits apply in CYP2D6 poor metabolisers and when strong inhibitors are co-prescribed.
