Aminophylline Injection is a fast-acting bronchodilator used to treat acute bronchospasm associated with asthma, chronic obstructive pulmonary disease (COPD), and other respiratory conditions. It works by relaxing the muscles of the airways and improving airflow to the lungs.

Aminophylline is a complex of theophylline with ethylenediamine, the ethylenediamine improving aqueous solubility sufficiently for intravenous administration. It acts as a non-selective phosphodiesterase inhibitor and adenosine receptor antagonist, producing bronchodilation together with respiratory stimulant effects. It is used in acute severe bronchospasm where inhaled therapy has proved insufficient, and in neonatal apnoea of prematurity.

The defining clinical constraint is its narrow therapeutic index. The gap between therapeutic and toxic serum concentrations is small, and toxicity manifests as arrhythmias and seizures, which may occur without preceding minor symptoms. Serum level monitoring is required. Clearance varies substantially with age, smoking status, hepatic function and concurrent medication — fluoroquinolones, macrolides and cimetidine among many others reduce clearance materially. This is why aminophylline has been displaced from first-line use in most guidelines despite remaining effective.

Sourcing notes. Despite that displacement, aminophylline remains a substantial tender item in markets where cost governs formulary choice and where nebulised therapy is less accessible. It is inexpensive and widely manufactured. Manufacture requires sterile small-volume parenteral approval at the specific plant. Confirm whether the requirement is aminophylline or theophylline, and on what basis strength is expressed — aminophylline is approximately eighty percent theophylline by weight, and specifications that do not state the basis are a recurring source of dosing error.
