Riociguat (Adempas®) is a soluble guanylate cyclase (sGC) stimulator used to treat pulmonary arterial hypertension (PAH) and chronic thromboembolic pulmonary hypertension (CTEPH). It works by relaxing blood vessels in the lungs, improving blood flow and reducing pulmonary pressure. Available as oral film-coated tablets in 0.5 mg to 2.5 mg strengths, it is taken under medical supervision.

Product Description:

Riociguat is a soluble guanylate cyclase stimulator, and its mechanism is distinct from every other pulmonary vasodilator class. It acts on sGC by two routes — sensitising the enzyme to endogenous nitric oxide, and stimulating it directly and independently of NO. That dual action matters in pulmonary hypertension, where nitric oxide availability is often impaired and NO-dependent therapies correspondingly limited.

It is approved for pulmonary arterial hypertension and, importantly, for chronic thromboembolic pulmonary hypertension, where it remains one of very few pharmacological options for patients who are inoperable or who have persistent disease after pulmonary endarterectomy. It is dosed three times daily and titrated individually from 0.5 or 1 mg to a maximum of 2.5 mg according to blood pressure tolerance.

Two absolute contraindications must be carried in labelling. Riociguat is contraindicated in pregnancy because of demonstrated embryo-foetal toxicity, and in the United States is subject to a restricted distribution programme requiring pregnancy testing and contraception. It is also contraindicated with PDE5 inhibitors and with nitrates in any form, as the combination produces severe hypotension.

Sourcing notes. Patent status varies by territory — generic riociguat is available in some markets and not others — so verify freedom to operate in the destination market specifically. Where a restricted distribution programme applies, ordinary import channels are not available and supply must run through the authorised route.
