Guaifenesin is a well-known expectorant used to loosen and thin mucus in the respiratory tract, making coughs more productive and helping relieve chest congestion associated with common cold, bronchitis, or other respiratory conditions.

Guaifenesin is an expectorant, and the distinction between an expectorant and a mucolytic matters when specifying a product. A mucolytic chemically breaks down mucus structure; an expectorant increases respiratory tract fluid volume, thinning secretions by dilution and making cough more productive. Guaifenesin does the latter, and does so only at adequate doses with adequate fluid intake.

It is among the most widely sold over-the-counter respiratory products globally, supplied as immediate-release tablets and syrup and as 600 and 1200 mg extended-release tablets, frequently in combination with dextromethorphan, pseudoephedrine or an antihistamine.

Its evidence base is modest. Guaifenesin is safe and widely used, but demonstrated benefit over adequate hydration alone is limited, and several regulators have taken a sceptical position on efficacy claims for the class.

Sourcing notes. Volumes are large and pricing is highly competitive, so the commercial questions are formulation and classification rather than availability. Extended-release tablets are a genuinely different manufacturing proposition from immediate-release and require dissolution profile validation. Where the product is a combination, the additional components govern registrability — a pseudoephedrine-containing combination brings precursor-control obligations, and codeine-containing combinations are restricted or prohibited for cough indications in a growing number of markets.
