The typhoid vaccine is given to prevent typhoid infection. Typhoid (also called typhoid fever) is a disease caused by bacteria. It is usually spread through food or water contaminated with bowel movement from an infected person. It can also spread through close contact with an infected person.

Typhoid vaccines protect against Salmonella Typhi, which causes an estimated nine million cases and over a hundred thousand deaths a year, concentrated in South Asia and sub-Saharan Africa. Three types exist. The Vi capsular polysaccharide vaccine, given by injection, protects for about three years in people over two. The live attenuated oral vaccine is taken as capsules over several days. The newer Vi conjugate vaccine links the polysaccharide to a carrier protein, which produces immune memory, works in infants from six months and protects for considerably longer.

The conjugate vaccine is the significant development, because it can be given in routine infant immunisation rather than only to travellers and outbreak contacts, and WHO has recommended its introduction in high-burden countries. That matters increasingly as extensively drug-resistant typhoid spreads, narrowing treatment options to a small number of antibiotics.

Sourcing notes. The three vaccine types are different products with different ages, schedules and durations, and an enquiry must state which. Polysaccharide and conjugate injectables require 2 to 8°C cold chain and must not be frozen; the oral live vaccine has its own handling requirements. Conjugate vaccine demand is programme-driven and WHO prequalification governs eligibility for UNICEF and Gavi-funded procurement.
