Yellow fever vaccine protects against a severe infectious disease called yellow fever. Yellow fever is caused by a flavivirus present in monkeys that can be transmitted to humans through an infected mosquito bite. This infection is characterized by fever lasting for a week with headache, muscle pain, slow heart rate, etc.

Yellow fever vaccine is a live attenuated virus, derived from the 17D strain developed in the 1930s. It is among the most effective vaccines in existence: a single dose produces protective antibody in around ninety-nine percent of recipients within thirty days, and WHO now regards that single dose as conferring lifelong protection, having removed the ten-year booster requirement from the International Health Regulations.

It is used in routine immunisation in endemic countries across Africa and South America, in outbreak response, and for travellers, for whom proof of vaccination on an International Certificate is a legal entry requirement in many countries. Serious adverse events are rare but real: vaccine-associated viscerotropic and neurotropic disease, occurring more often at the extremes of age and in the immunosuppressed, which is why the vaccine is contraindicated in significant immunodeficiency, in infants under six months, and used with caution over sixty. Egg allergy is a contraindication, as the vaccine is egg-grown.

Global production capacity is limited and concentrated in a handful of WHO-prequalified manufacturers, and outbreak demand has repeatedly outstripped supply, forcing fractional dosing.
