This capsule combines silymarin, the milk thistle extract, with vitamins B2, B3, B5, B9 (folic acid) and B12, vitamin E and zinc. Silymarin has antioxidant and membrane-stabilising activity in laboratory studies of liver cells. The B vitamins are cofactors for energy metabolism in the liver and are often depleted in people with chronic liver disease or heavy alcohol use. Vitamin E is an antioxidant, and zinc is frequently low in cirrhosis.

Replacing vitamins and zinc that are genuinely deficient is reasonable in liver disease. Vitamin E has trial evidence in non-diabetic adults with fatty liver disease, at doses higher than usually found in combination capsules, and with some long-term safety questions. Silymarin has not shown consistent benefit on clinical outcomes. The product does not replace medical assessment and treatment of liver disease. It is well tolerated; milk thistle can cause allergy in people sensitive to plants of the daisy family.
