This tablet combines silymarin, N-acetylcysteine, L-carnitine L-tartrate, L-ornithine, inositol, choline bitartrate and coenzyme Q10, and is marketed for liver support. Silymarin is the flavonolignan complex from milk thistle seeds, with antioxidant activity in laboratory studies. N-acetylcysteine replenishes glutathione — it is the antidote for paracetamol overdose, at much higher doses. Choline and inositol are involved in fat transport from the liver. L-ornithine takes part in ammonia detoxification, and L-carnitine in fatty acid metabolism.

Clinical trials of silymarin in alcohol-related liver disease, hepatitis and fatty liver disease have not shown consistent benefits on meaningful outcomes, and the other ingredients have limited evidence as liver supplements. The product is not a treatment for liver disease, where weight loss, alcohol cessation and medical care are what change outcomes. Most ingredients are well tolerated. Milk thistle can cause allergy in people sensitive to the daisy family, and CoQ10 can reduce the effect of warfarin.

Sourcing notes. Buyers should specify the dose of each component when enquiring. Silymarin extracts should be standardised, typically to 70–80% silymarin, with the analytical method stated. Several ingredients are hygroscopic, which affects tablet stability. Liver claims are restricted in most regulated markets.
