Product Description:

Rasburicase is a recombinant version of urate oxidase, an enzyme humans lost through mutation millions of years ago. Every other mammal converts uric acid into allantoin, which is far more soluble and easily excreted; people cannot, which is why uric acid crystallises in joints and kidneys. Supplying the missing enzyme converts circulating urate within hours, rather than merely slowing its production as allopurinol does.

Its use is in tumour lysis syndrome — the flood of intracellular contents released when a large, rapidly dividing tumour is treated and dies at once. Uric acid precipitates in the renal tubules and causes acute kidney injury, often alongside dangerous potassium and phosphate rises. Rasburicase both prevents and treats this, and is given intravenously before or with the first cycle in high-risk leukaemias and lymphomas. It must not be used in glucose-6-phosphate dehydrogenase deficiency, where it causes severe haemolysis and methaemoglobinaemia.

Sourcing notes. A biologic, supplied as powder with a specific diluent, requiring cold chain throughout. G6PD deficiency is common across the Mediterranean, Africa, the Middle East and South Asia, so markets in those regions need screening capacity alongside the product. Blood samples for urate measurement must be handled on ice.
