Product Description:

Megestrol acetate is a synthetic progestin with two quite separate uses that depend on dose. At high doses it suppresses gonadotrophin release and acts directly on hormone-dependent tumours, reducing oestrogen drive. At any dose it stimulates appetite through a mechanism that is still not fully explained but appears to involve neuropeptide Y in the hypothalamus and suppression of inflammatory cytokines.

The 40 mg tablet strength serves the oncology indications: hormone-dependent breast cancer and endometrial carcinoma, usually after other endocrine therapy. For appetite stimulation in cancer cachexia and AIDS-related wasting, much higher doses are needed and an oral suspension is normally used instead. Weight gained is largely fat rather than lean mass, and survival is not improved. Thromboembolism is the significant risk and is the reason it is used cautiously in immobile patients. Adrenal suppression can follow prolonged use and withdrawal must be gradual.
