Thalidomide is an immunomodulatory drug used for multiple myeloma and for erythema nodosum leprosum, an inflammatory complication of leprosy. It causes severe, life-threatening birth defects: a single dose in pregnancy can harm the unborn child, so it is supplied only under a controlled pregnancy prevention programme.

Thalidomide is the most notorious drug in pharmaceutical history and, in a controlled setting, a genuinely useful one. Withdrawn in 1961 after causing severe limb malformations in thousands of infants, it returned decades later when the same antiangiogenic and immunomodulatory properties responsible for that harm proved effective against cancer. It inhibits new blood vessel formation, modulates tumour necrosis factor alpha, and stimulates T-cell and natural killer cell activity.

It is used in multiple myeloma, usually with dexamethasone or melphalan, and in erythema nodosum leprosum, where it controls a severely painful inflammatory complication of leprosy. Peripheral neuropathy is common, cumulative and often irreversible, requiring regular neurological assessment. Venous thromboembolism risk is high enough that prophylactic anticoagulation is standard in myeloma. Sedation, constipation and rash occur.
