Rabeprazole is a proton pump inhibitor. In the acidic canaliculi of the stomach's parietal cells it converts to an active form that binds irreversibly to the hydrogen-potassium ATPase, the pump responsible for the final step of acid secretion. Because the binding is permanent, acid suppression lasts until new pumps are made, well beyond the drug's short time in the blood. Rabeprazole activates faster than other PPIs and relies less on CYP2C19, so its effect varies less between patients with different metabolising genotypes.

It is used for gastro-oesophageal reflux disease, erosive oesophagitis, duodenal and gastric ulcers, as part of Helicobacter pylori eradication regimens, and in Zollinger-Ellison syndrome. It is taken before a meal, since the pumps must be active for the drug to bind them. Long-term use is associated with low magnesium, vitamin B12 deficiency, Clostridioides difficile infection and possibly fracture risk, so treatment is reviewed periodically rather than continued indefinitely by default.

Sourcing notes. The gastro-resistant (enteric-coated) coating is essential, because the drug is destroyed by stomach acid, so coating integrity and acid-resistance testing are the critical quality attributes. Off patent with wide generic availability.
