Protionamide is a drug used in the treatment of tuberculosis. It has also been tested for use in the treatment of leprosy.

Product Description:

Protionamide (prothionamide) is a second-line antituberculosis agent, a thioamide structurally related to ethionamide and sharing its mechanism: inhibition of mycolic acid synthesis, disrupting formation of the mycobacterial cell wall. It is a prodrug requiring activation by the mycobacterial EthA enzyme, and cross-resistance with ethionamide is essentially complete — a resistance testing result for one applies to the other.

Its role is in the treatment of multidrug-resistant tuberculosis, as part of a combination regimen and never as monotherapy. It has also been used in multibacillary leprosy regimens. Standard presentation is the 250 mg tablet.

Tolerability is the practical limitation. Gastrointestinal intolerance is common and frequently dose-limiting, hypothyroidism occurs with prolonged use and requires monitoring, and hepatotoxicity is recognised. Adherence over the extended courses that MDR-TB demands is correspondingly difficult.

Sourcing notes. Second-line TB medicines move largely through public health procurement rather than commercial channels — national TB programmes, the Global Drug Facility and donor-funded mechanisms — and that shapes both pricing and the documentation required. WHO prequalification or stringent regulatory authority approval is frequently a condition of eligibility for these tenders, and is a more demanding threshold than national GMP alone.
