Product Description:

Tiotropium bromide is a long-acting muscarinic antagonist and was the first once-daily bronchodilator to establish that class in COPD maintenance treatment. It binds M3 muscarinic receptors on airway smooth muscle with prolonged kinetics, dissociating slowly enough to sustain bronchodilation over twenty-four hours, and it reduces exacerbation frequency as well as improving airflow. It is also approved as add-on maintenance therapy in asthma inadequately controlled on inhaled corticosteroids.

The device is inseparable from the product. Tiotropium is supplied through two quite different delivery systems: a dry-powder capsule inhaler, in which the capsule is loaded into a breath-actuated device and is never to be swallowed, and a soft-mist inhaler delivering an aqueous solution. Nominal strengths differ between them — typically 18 mcg in the capsule system against 2.5 mcg per actuation in the soft-mist device — and they are not interchangeable. A specification stating only "tiotropium 18 mcg" is describing the capsule product, and confusing the two is a genuine dispensing hazard.

Sourcing notes. Device documentation forms part of the registration file in most markets, and device supply commonly has longer lead times than the formulation. Confirm which delivery system is required and whether the manufacturer holds the device approval for the destination market, not merely the drug approval. Patient counselling on the capsule product — that capsules are for inhalation only — should be reflected in the pack.
