Levodropropizine is a peripherally acting antitussive, and that mechanism is its defining feature. Rather than suppressing the cough centre in the brainstem, it acts on sensory C-fibre receptors within the respiratory tract, modulating the afferent limb of the cough reflex. Because it does not act centrally, it avoids the sedation, respiratory depression and dependence potential of opioid antitussives — a meaningful advantage in paediatric use and in elderly patients.

Comparative studies against central antitussives have shown comparable cough suppression with significantly less somnolence. It is indicated for dry, non-productive cough, and is supplied as 60 mg tablets and as syrup at 30 mg per 5 mL.

Sourcing notes. Levodropropizine originated in Italy and its use remains concentrated in Italy, South Korea, and parts of Latin America and Southeast Asia. It has not been approved in the United States or United Kingdom. Registration precedent therefore determines which markets are realistic, and introducing it where none exists is a substantial undertaking rather than a filing. Italian and Korean manufacturers dominate supply, with growing Indian capability serving export markets. The syrup presentation carries most paediatric volume and requires stability data matched to the destination climatic zone.
