Palbociclib is a type of cancer growth blocker that targets the proteins cyclin dependent kinase 4 and 6 (CDK4 and CDK6) on cancer cells. CDK 4 and CDK 6 are proteins that stimulate cancer cells to grow and divide. Palbociclib works by blocking these proteins. It aims to slow or stop the growth of the cancer.

Palbociclib inhibits cyclin-dependent kinases 4 and 6, the enzymes that release the brake holding a cell at the boundary between the resting phase and DNA synthesis. In hormone receptor-positive breast cancer, oestrogen signalling drives that machinery, so blocking the kinases arrests the cell cycle before division and restores sensitivity to endocrine therapy. It was the first drug of its class and it roughly doubled progression-free survival when added to hormonal treatment.

It is used with an aromatase inhibitor or with fulvestrant in hormone receptor-positive, HER2-negative advanced breast cancer, taken for three weeks of every four. Neutropenia is very common but behaves differently from chemotherapy-induced neutropenia — it is a reversible arrest of marrow cell cycling rather than cell destruction, recovers promptly in the off week, and carries a much lower infection risk than the counts suggest. Fatigue, stomatitis, alopecia and nausea occur; counts are checked before each cycle.
