Olaparib is used as maintenance treatment in patients with advanced ovarian cancer, fallopian tube cancer, or primary peritoneal cancer with a certain type of inherited (germline) or acquired (somatic) abnormal BRCA gene.

Product Description:

Olaparib was the first PARP inhibitor approved anywhere and remains the most widely used. It blocks the enzyme that repairs single-strand DNA breaks and additionally traps it on the DNA strand, which is more damaging than simply inhibiting it. Cells that have lost homologous recombination — through BRCA1 or BRCA2 mutation or an equivalent defect — cannot deal with the resulting double-strand breaks and die, while cells with intact repair survive.

It is used as maintenance in BRCA-mutated advanced ovarian, fallopian tube and primary peritoneal cancer after platinum chemotherapy, in HER2-negative metastatic breast cancer with germline BRCA mutations, in metastatic pancreatic cancer, and in BRCA-mutated castration-resistant prostate cancer. Nausea, fatigue and anaemia are the common toxicities, the anaemia often requiring transfusion. Myelodysplastic syndrome and acute myeloid leukaemia are uncommon but serious long-term risks, and pneumonitis has been reported.
