Product Description:

Nortriptyline is a secondary amine tricyclic antidepressant and the active metabolite of amitriptyline. It blocks reuptake of noradrenaline strongly and serotonin more weakly, and in chronic pain that noradrenergic action is the relevant one: it strengthens the descending inhibitory pathways that damp pain signals in the spinal cord. The effect on pain is independent of any effect on mood and appears at lower doses than antidepressant treatment requires.

It is used for neuropathic pain — diabetic neuropathy, post-herpetic neuralgia, and other nerve pain — and for depression, and off label for migraine prevention. Compared with amitriptyline it causes less drowsiness, dry mouth, constipation and postural dizziness, which makes it easier to tolerate, particularly in older patients. Tricyclics affect cardiac conduction, so caution applies after myocardial infarction and with arrhythmia. It is metabolised by CYP2D6, and blood levels vary widely between people; measuring them is useful when response is poor. It must not be combined with MAO inhibitors.
