Butorphanol is a synthetic opioid analgesic that acts as a mixed agonist–antagonist at opioid receptors—stimulating kappa receptors and partially antagonizing mu receptors. It provides effective pain relief with a lower risk of respiratory depression compared to pure opioid agonists. Commonly used in surgery, migraine treatment, and labor pain management.

Butorphanol is a mixed opioid agonist-antagonist: an agonist at kappa receptors and a partial agonist or antagonist at mu receptors. That combination gives useful analgesia with a ceiling on respiratory depression, which is its main safety advantage over pure mu agonists such as morphine. The same pharmacology means it can precipitate withdrawal in a patient already dependent on a full opioid agonist, and adding it to morphine can reduce morphine's effect.

It is used for moderate to severe pain, including in labour and after surgery, and the nasal spray is used for acute migraine, where it offers a non-oral option when vomiting prevents tablets. Kappa activation causes sedation and, in some patients, dysphoria or hallucinations, which limits acceptability. Dizziness and nausea are common. Dependence can develop, particularly with the nasal spray, and it is a controlled substance in most markets.

Sourcing notes. Registration has narrowed, and in several markets the nasal spray has been discontinued, leaving the injection. Butorphanol is also widely used in veterinary anaesthesia, a separate registration and a substantial part of the market. Controlled-substance permits apply in most destinations; confirm scheduling locally before quoting.
