Nerandomilast is a selective phosphodiesterase-4B (PDE4B) inhibitor approved for idiopathic pulmonary fibrosis (IPF) and progressive pulmonary fibrosis (PPF). It slows the decline in lung function by reducing inflammatory and fibrotic signalling in the lung, and is taken orally once daily. It is not indicated for COPD.

Nerandomilast is an oral preferential inhibitor of phosphodiesterase 4B, developed for pulmonary fibrosis. PDE4B is the isoform most associated with inflammatory and fibrotic signalling, and targeting it preferentially rather than inhibiting PDE4 broadly was intended to retain antifibrotic effect while reducing the gastrointestinal effects that limit non-selective PDE4 inhibitors.
The US FDA approved it in September 2025 for idiopathic pulmonary fibrosis, and subsequently for progressive pulmonary fibrosis on the basis of the FIBRONEER-ILD trial. Japan has approved both indications. It is the first genuinely new mechanism approved in IPF in over a decade, and it slows the decline in forced vital capacity rather than reversing established fibrosis.
A correction worth stating plainly: nerandomilast is not a COPD treatment and has not been approved for COPD. It is a pulmonary fibrosis product, and the distinction matters because the prescriber, the patient population and the procurement route are entirely different.
Sourcing notes. This is a newly approved, on-patent innovator product with a single manufacturer. No generic exists or lawfully can for years, and any offer of generic nerandomilast should be refused. Approvals outside the US and Japan were still progressing through 2026, so availability is strictly market-dependent. For patients in markets without approval, named-patient import is the mechanism, requiring physician documentation and regulatory authorisation in the destination country.
