Levalbuterol is the R-enantiomer of albuterol, isolated from the racemic mixture. Albuterol as conventionally supplied contains equal parts R- and S-enantiomer; the R form carries essentially all the bronchodilator activity, while the S form was historically assumed inert and later suggested to contribute to airway irritation. Levalbuterol was developed on the proposition that removing the S-enantiomer would improve the therapeutic ratio.
In practice the clinical advantage has proved modest. Comparative studies have not consistently demonstrated meaningful superiority over racemic albuterol at equivalent bronchodilator dose, and guidelines generally treat them as therapeutically similar. Levalbuterol is materially more expensive.

Sourcing notes. Chiral separation makes the API more costly to produce than racemic albuterol, and that difference persists through to finished product. Where a tender specifies levalbuterol, confirm whether the requirement is genuinely for the single enantiomer or whether racemic albuterol would satisfy it — the cost difference is substantial and the clinical case for the premium is not strong. Where levalbuterol is genuinely required, note that dose equivalence is not one-to-one with racemic albuterol and specifications should state which is meant.
