Maribavir (Livtencity) is an antiviral used to treat cytomegalovirus (CMV) infection in patients who have had an organ or stem-cell transplant and whose CMV infection is resistant or refractory to standard treatments. It works by inhibiting the viral protein kinase pUL97, preventing CMV DNA replication, encapsidation, and nuclear egress, thereby stopping the virus from multiplying.

Product Description:
Maribavir inhibits the cytomegalovirus enzyme UL97, a protein kinase the virus needs for DNA packaging and for exporting new viral particles from the nucleus. Because it blocks a different target from the DNA polymerase that ganciclovir, foscarnet and cidofovir attack, it remains active against virus resistant to all three — which is its reason for existing.
It is used for post-transplant cytomegalovirus infection that is refractory to or resistant to standard treatment, in adults and adolescents, as an oral tablet twice daily. In its pivotal trial it cleared viraemia more often than investigator-chosen therapy, and with far less toxicity: no significant myelosuppression and no nephrotoxicity, which is the central advantage in transplant patients already dealing with both. Taste disturbance — a persistent metallic taste — is the commonest adverse effect. It antagonises ganciclovir and valganciclovir, so they must not be given together, and it interacts with immunosuppressants, whose levels need monitoring.
