Product Description:
Raltegravir was the first integrase strand transfer inhibitor. HIV must insert its DNA into the host genome to establish infection, and the viral integrase enzyme performs that step; raltegravir binds the enzyme-DNA complex and blocks strand transfer, so the viral DNA is never integrated. Because no human enzyme does this, the class is unusually well tolerated.
It is used with other antiretrovirals for HIV-1 in adults and children from birth, including in pregnancy and in post-exposure prophylaxis, where its rapid viral load reduction is valued. It has few drug interactions because it is metabolised by glucuronidation rather than cytochrome enzymes, which makes it useful in patients on tuberculosis treatment or complex co-medication. Its drawbacks are a lower genetic barrier to resistance than the newer integrase inhibitors, and twice-daily dosing for the standard formulation. Rash and, rarely, severe hypersensitivity occur; creatine kinase rises and myopathy have been reported.
