Product Description:

This suspension combines three agents. Magaldrate is an aluminium-magnesium hydroxide sulfate complex that neutralises acid gradually as it converts to aluminium and magnesium hydroxides in the stomach. Methylpolysiloxane (dimethicone) is an antifoaming agent that relieves trapped gas. Domperidone is a peripherally acting dopamine D2 antagonist that speeds gastric emptying and reduces nausea; it does not cross the blood-brain barrier readily, so extrapyramidal effects are uncommon.

It is used for dyspepsia with acidity, bloating and nausea. The pharmacology of the combination is questionable: domperidone needs an acidic stomach for good absorption, so co-formulating it with an antacid can reduce how much is absorbed. Domperidone's main safety concern is QT prolongation and ventricular arrhythmia, particularly above 30 mg a day, in older patients and with CYP3A4 inhibitors. Antacid components reduce absorption of tetracyclines, fluoroquinolones, iron and levothyroxine.

Sourcing notes. In 2014 the European Medicines Agency restricted domperidone to relief of nausea and vomiting, at the lowest dose for no more than a week, because of cardiac risk. This combination, intended for ongoing dyspepsia, will not be registrable in most regulated markets. It is registered mainly in South Asia and some African markets. Confirm the destination regulator accepts it before any quotation is prepared.
