Product Description:

Aluminium hydroxide gel is a colloidal suspension of hydrated aluminium oxide used medically as an antacid and as a phosphate binder. As an antacid it neutralises gastric acid more slowly than magnesium or sodium salts but acts for longer. As a phosphate binder it combines with dietary phosphate in the gut to form insoluble aluminium phosphate, which is excreted, lowering serum phosphate in chronic kidney disease.

Constipation is the characteristic adverse effect, and high doses cause phosphate depletion in people with normal kidneys. The more serious concern is aluminium accumulation in renal failure, which caused encephalopathy, osteomalacia and anaemia in dialysis patients before the risk was recognised. For that reason it is now used as a phosphate binder only for short periods, with calcium-based and non-calcium binders preferred for long-term use. Like other antacids it reduces absorption of tetracyclines, fluoroquinolones, iron and levothyroxine.

Sourcing notes. Aluminium hydroxide is made in several grades: pharmaceutical antacid gel, vaccine-adjuvant grade and industrial grade. They are not interchangeable, and the buyer must specify which is required. For a pharmaceutical gel, acid-neutralising capacity, reaction rate, heavy-metal limits and microbial quality are the core specifications, and the relevant pharmacopoeial monograph should be named.
