Product Description:

Lumateperone acts across three systems at once, which is unusual. It is a potent 5-HT2A antagonist, a dopamine receptor phosphoprotein modulator — acting as a presynaptic partial agonist and postsynaptic antagonist at D2, so it modulates rather than simply blocks dopamine — and it inhibits serotonin reuptake and affects glutamate signalling. The result is antipsychotic activity at a level of D2 occupancy well below what typical antipsychotics require.

It is used for schizophrenia in adults and for depressive episodes in bipolar I and II disorder, alone or with lithium or valproate. Its clinical argument is tolerability: in trials, weight gain, metabolic disturbance, prolactin elevation and movement disorders were close to placebo levels — the adverse effects that most often lead patients to stop other antipsychotics. Sedation and dry mouth are the common complaints. It carries the class warning about increased mortality in elderly patients with dementia-related psychosis. It is a CYP3A4 substrate, so strong inhibitors and inducers matter.
