Amoxapine is a tricyclic antidepressant (TCA) used primarily to treat major depressive disorder, especially in patients with depressive symptoms that may include anxiety or mild psychotic features. It works by balancing neurotransmitters in the brain, such as norepinephrine and serotonin, to improve mood, energy, and interest in daily activities.

Amoxapine is a tricyclic antidepressant with a difference: it is a metabolite of the antipsychotic loxapine, and it blocks dopamine D2 receptors as well as inhibiting noradrenaline and serotonin reuptake. That dual action gives it antipsychotic as well as antidepressant activity, which is why it has been used for psychotic depression, where both symptom clusters must be treated.

It is used for major depressive disorder, including depression with psychotic or anxious features. The D2 blockade is also its main drawback: unlike other tricyclics it can cause extrapyramidal effects — parkinsonism, akathisia and, with long-term use, tardive dyskinesia — and neuroleptic malignant syndrome has been reported. It raises prolactin. Otherwise it shares the tricyclic profile: sedation, dry mouth, constipation, postural hypotension, weight gain, lowered seizure threshold and cardiac conduction effects, with danger in overdose. It must not be combined with MAO inhibitors.

Sourcing notes. Off patent with a narrow supplier base, registered mainly in the US and a few other markets. It is prescribed rarely, so demand is small and specialist. For most buyers a combination of an SSRI with an antipsychotic is the locally registered equivalent, which is worth knowing when a request is really about psychotic depression.
