Product Description:

Lactitol is a sugar alcohol made from lactose that is not absorbed in the small intestine. In the colon, bacteria ferment it into short-chain fatty acids, which acidify the bowel and draw in water by osmosis, softening stool and increasing bowel movements. It works in the same way as lactulose but is supplied as a powder, which many patients find easier to take than a sweet syrup.

In kidney disease constipation is common and consequential. Fluid restriction, phosphate binders — particularly calcium and iron-based ones — and reduced intake of high-fibre fruit and vegetables because of their potassium content all contribute. Constipation in turn makes peritoneal dialysis catheter function worse and is associated with higher potassium absorption. Osmotic laxatives are generally preferred in this group because stimulant laxatives are less suitable long term. Bloating and wind are common at first. Lactitol is not suitable in galactosaemia or bowel obstruction.

Sourcing notes. Lactitol is registered as a medicine for constipation in Europe and the US and is also a food ingredient, so the grade and intended use must be clear, with the pharmacopoeial monograph named. The powder is hygroscopic and needs moisture-protective sachets.
