Alosetron is a potent, selective 5-HT3 receptor antagonist. Serotonin released from enterochromaffin cells acts on 5-HT3 receptors on enteric neurons to speed colonic transit, increase secretion and heighten visceral sensitivity. Blocking those receptors slows transit, firms stool and reduces urgency and abdominal pain, which is why it is reserved for women with severe diarrhoea-predominant irritable bowel syndrome who have not responded to conventional treatment.

Its history shapes its use. It was withdrawn in the US in 2000, months after launch, following reports of ischaemic colitis and serious complications of constipation, some fatal. It returned in 2002 under a restricted programme with a lower starting dose. That programme was removed in 2023, but the boxed warning remains: treatment stops immediately if constipation or signs of ischaemic colitis such as rectal bleeding appear, and it is not started in patients who are constipated.
