Product Description:
Fosaprepitant dimeglumine is a water-soluble phosphorylated prodrug of aprepitant. Within about 30 minutes of infusion it is converted to aprepitant, which blocks neurokinin-1 (NK1) receptors for substance P in the brain areas that drive vomiting. This page covers the 150 mg single-dose vial of freeze-dried powder. The intravenous form lets oncology day units give the NK1 component once, with the rest of the premedication, instead of a three-day oral capsule course.
It is used with a 5-HT3 antagonist and dexamethasone to prevent acute and delayed nausea and vomiting with highly emetogenic chemotherapy, such as cisplatin regimens, and with moderately emetogenic chemotherapy. The dose is infused over 20 to 30 minutes, about half an hour before chemotherapy. Aprepitant moderately inhibits CYP3A4, so the oral dexamethasone dose is usually reduced. It can weaken hormonal contraceptives for several weeks, and patients on warfarin need INR checks. Infusion-site reactions, including thrombophlebitis, and hypersensitivity, including anaphylaxis, have been reported.
Sourcing notes. Generic versions are registered in many markets, usually on a dossier showing equivalent aprepitant exposure. The powder is reconstituted with sodium chloride 0.9% and diluted before infusion. Check the label for storage, as the originator vial is kept at 2-8°C while some generics differ. Ask for a CoA per batch with assay, impurities, sterility and endotoxin, a GMP certificate and CoPP, and zone IVb stability data for hot climates. It is prescription-only. Give vial numbers, required shelf life on arrival and whether you need a CTD or ACTD dossier.
