Exemestane is an aromatase inhibitor used in the treatment of hormone receptor–positive breast cancer in postmenopausal women. It works by blocking the enzyme aromatase, which converts androgens into estrogens, thereby reducing estrogen levels and slowing cancer cell growth. Exemestane is usually prescribed after other treatments like tamoxifen.

Exemestane is a steroidal aromatase inhibitor, and the distinction from the non-steroidal drugs in the class is not academic. Because it resembles the enzyme's natural substrate, it binds irreversibly and destroys the enzyme rather than merely occupying it — which is why it can still work in women whose disease has progressed on anastrozole or letrozole. Oestrogen production in postmenopausal women depends on this enzyme in fat, muscle and the tumour itself.

It is used for hormone receptor-positive early breast cancer, usually after two to three years of tamoxifen, and for advanced disease after progression on other endocrine therapy. It is also used with ovarian suppression in premenopausal women. Adverse effects reflect oestrogen deprivation: hot flushes, arthralgia severe enough to cause many women to stop treatment, fatigue, and progressive bone mineral density loss requiring baseline densitometry and monitoring. Its mild androgenic structure can cause acne and hair thinning.
