Product Description:

This product is a delivery system as much as a drug. Mitomycin is an old cytotoxic antibiotic that cross-links DNA; the difficulty in upper tract urothelial cancer has never been the drug but the plumbing, because anything instilled into the renal pelvis drains away down the ureter within minutes. Here the mitomycin is mixed into a reverse-thermal hydrogel: liquid when cold, semi-solid at body temperature. Instilled cold through a catheter, it sets in place and then dissolves slowly in urine over hours.

It is used for low-grade upper tract urothelial cancer, given as weekly instillations. Its significance is organ preservation — the standard alternative is removal of the kidney and ureter, and for patients with a single kidney, bilateral disease or poor renal function that is a catastrophic operation to avoid. Ureteric obstruction and stenosis are the characteristic complications and require surveillance. Flank pain, urinary tract infection, haematuria and fatigue occur.
