Betahistine is a histamine analogue medicine that is used to treat symptoms of Ménière's syndrome such as dizziness (vertigo), ringing in the ears (tinnitus), loss of hearing and nausea. This medicine works by improving blood flow in the inner ear. This lowers the buildup of pressure.

Betahistine is a histamine analogue: a weak agonist at H1 receptors and a much stronger antagonist at H3 receptors. Blocking H3 autoreceptors increases histamine turnover in the brainstem vestibular nuclei, and the H1 effect dilates the microcirculation of the inner ear. The working theory is that improved blood flow in the stria vascularis reduces endolymphatic pressure, which is the abnormality thought to underlie Ménière's disease.

It is used to reduce the frequency and severity of attacks of vertigo, tinnitus and hearing loss in Ménière's disease, and in some markets for other forms of vestibular vertigo. Evidence is mixed: it has been used widely across Europe and Asia for decades, but a large placebo-controlled trial in Germany found no benefit over placebo in Ménière's disease, and it is not approved in the United States. It is generally well tolerated, with headache and gastrointestinal upset being the usual complaints. As a histamine analogue it is used with caution in asthma and peptic ulcer disease.
