Alectinib is an oral anticancer medication used for the treatment of ALK-positive non-small cell lung cancer (NSCLC). It works as a tyrosine kinase inhibitor (TKI) that specifically targets and blocks the activity of the anaplastic lymphoma kinase (ALK) protein, which promotes the growth of cancer cells. By inhibiting ALK, alectinib helps slow or stop the spread of cancer.

Product Description:

Alectinib inhibits anaplastic lymphoma kinase, the fusion protein produced when the ALK gene rearranges and drives tumour growth. It is a second-generation ALK inhibitor, designed to overcome the resistance mutations that emerge on crizotinib, and — importantly — it is not a substrate for the P-glycoprotein pump that keeps many drugs out of the brain, so it reaches therapeutic concentrations in the central nervous system.

It is used for ALK-positive non-small cell lung cancer, first-line and after crizotinib, and as adjuvant treatment after resection. Brain penetration matters here more than in most cancers: brain metastases occur in a large proportion of ALK-positive patients, and in trials alectinib both treated existing central nervous system disease and delayed its appearance, which is why it displaced crizotinib as standard first-line therapy. Myalgia with raised creatine kinase, anaemia, constipation, oedema and photosensitivity are common; liver enzymes are monitored.

Sourcing notes. Originator product, on patent, no generic. ALK testing — by immunohistochemistry, FISH or next-generation sequencing — is a prerequisite, and only about three to five percent of lung adenocarcinomas are ALK-positive, so the addressable population is small but the treatment duration is long.
