Product Description:

Niraparib inhibits poly-ADP-ribose polymerase, the enzyme that repairs single-strand DNA breaks. Blocking it turns those breaks into double-strand breaks at the replication fork. A normal cell repairs those by homologous recombination; a cell with a BRCA mutation or another homologous recombination defect cannot, and dies. This is synthetic lethality — neither the drug alone nor the mutation alone is fatal, but together they are.

It is used as maintenance treatment after platinum-based chemotherapy in advanced ovarian, fallopian tube and primary peritoneal cancer, taken once daily. It is the PARP inhibitor most often used without requiring a BRCA result, since benefit extends beyond the mutated population, though it is greatest there. Thrombocytopenia is the characteristic toxicity and is related to body weight and baseline platelet count, which is why the starting dose is individualised. Anaemia, neutropenia, hypertension, fatigue, nausea and insomnia occur, and myelodysplastic syndrome is a rare long-term risk.
