Vincristine is a vinca alkaloid chemotherapy used in acute lymphoblastic leukaemia, Hodgkin and non-Hodgkin lymphoma, multiple myeloma, and childhood cancers including Wilms tumour, neuroblastoma and rhabdomyosarcoma. For intravenous use only: it is fatal if given by any other route, and is supplied for dispensing in a minibag rather than a syringe.

Product Description:

Vincristine binds tubulin and prevents microtubules from assembling, so the mitotic spindle cannot form and the dividing cell arrests and dies. Its value in combination regimens comes from an unusual property for a cytotoxic: at standard doses it causes almost no myelosuppression, so it can be added to marrow-suppressing drugs without compounding their effect. The dose-limiting toxicity is neurological instead.

It is a component of curative regimens in acute lymphoblastic leukaemia, Hodgkin and non-Hodgkin lymphoma, and the paediatric solid tumours — Wilms tumour, rhabdomyosarcoma, neuroblastoma, Ewing sarcoma — and is used in multiple myeloma regimens. Peripheral neuropathy is universal with continued treatment, beginning with loss of ankle reflexes and progressing to sensory loss, weakness and constipation from autonomic involvement. It is a severe vesicant. Doses are commonly capped at 2 mg regardless of body surface area.

Sourcing notes. Off patent and inexpensive, with demand concentrated in paediatric oncology. One safety requirement governs supply: vincristine is fatal if given into the spine, and international guidance requires it to be dispensed in a small infusion bag rather than a syringe.
