Valganciclovir is a prodrug of ganciclovir, an antiviral against cytomegalovirus. Ganciclovir itself is poorly absorbed by mouth, so it historically required intravenous infusion. Adding a valine ester allows intestinal transporters to carry it across the gut wall, after which esterases release ganciclovir, giving blood levels comparable to intravenous dosing from a tablet. Inside infected cells a viral kinase adds the first phosphate, which concentrates the active drug where the virus is.

It is used to prevent CMV disease after solid organ transplantation, particularly in high-risk combinations where the donor is CMV-positive and the recipient negative, and to treat CMV retinitis in advanced HIV. In transplant care it is standard prophylaxis for a set period after surgery. Its limiting toxicity is bone marrow suppression — neutropenia, anaemia and thrombocytopenia — so blood counts are monitored, and the dose must be adjusted carefully for renal function, which is frequently impaired in transplant recipients. It is teratogenic and carcinogenic in animals, so tablets must not be crushed and handling precautions apply.
