This tablet combines lactoferrin, disodium guanosine 5'-monophosphate and elemental iron. Lactoferrin is an iron-binding glycoprotein from milk that regulates iron absorption and handling and has anti-inflammatory activity. Pairing it with a smaller dose of elemental iron is intended to improve tolerance: much of the nausea and constipation from conventional ferrous salts comes from unabsorbed free iron in the gut.

Anaemia in chronic kidney disease has several causes — reduced erythropoietin production, absolute iron deficiency, and functional iron deficiency where inflammation traps iron in stores. Hepcidin, the hormone that blocks iron absorption and release, is raised in kidney disease, which is why oral iron often works poorly in these patients and intravenous iron is frequently needed. Lactoferrin has been studied as a gentler oral option, with small trials reporting comparable haemoglobin responses to ferrous salts and fewer gastrointestinal complaints, but the evidence is limited and it is not guideline-recommended. Iron status should be measured before and during treatment.
