Probiotics are live microorganisms intended to confer a health benefit when taken in adequate amounts. The genera most used are Lactobacillus, Bifidobacterium and the yeast Saccharomyces boulardii. Their proposed mechanisms are competitive exclusion of pathogens, strengthening of the intestinal barrier, production of short-chain fatty acids, and modulation of local immune signalling.

The single most important thing to understand about them is that effects are strain-specific, not species-specific. Evidence that one strain shortens antibiotic-associated diarrhoea says nothing whatever about a different strain of the same species. The best-supported uses are antibiotic-associated and infectious diarrhoea and some irritable bowel symptoms. They are generally well tolerated, but they are live organisms and are approached with caution in the severely immunocompromised, in critically ill patients and in anyone with a central venous catheter, where bacteraemia and fungaemia have been reported.

Sourcing notes. A meaningful enquiry needs the strain designations, the colony-forming unit count guaranteed at end of shelf life rather than at manufacture, and the storage conditions — many products require refrigeration and lose viability in warm transit. Buyers should ask for stability data. Regulated as a food supplement in most destinations.
