Product Description:

This injection combines mecobalamin (the active coenzyme form of vitamin B12), pyridoxine (B6), folic acid and niacinamide (B3), and is given to patients on haemodialysis. The rationale is loss: water-soluble vitamins are small enough to cross the dialyser membrane, so folate, B6, B12 and other B vitamins are removed at each session. Dietary restriction of potassium and phosphate further limits intake, since many vitamin-rich foods are high in both.

Supplementation of water-soluble vitamins is routine in dialysis programmes for this reason, usually orally, with injection used where absorption is poor or adherence is a problem. Folate, B6 and B12 also lower homocysteine, which is raised in kidney failure; trials of homocysteine lowering in dialysis patients have not shown reduced cardiovascular events, so supplementation is justified by replacing losses rather than by that hypothesis. Fat-soluble vitamins are a different matter and vitamin A in particular accumulates in kidney failure, so general multivitamins are not appropriate.

Sourcing notes. The amount of each vitamin per ampoule should be stated on any quotation. Mecobalamin is light-sensitive, so amber ampoules and light-protective packaging are required, and stability data should cover light exposure. Dialysis chains are the buyers, so volumes are predictable.
