Product Description:

Tobramycin is an aminoglycoside antibiotic that binds the bacterial 30S ribosomal subunit and causes misreading of messenger RNA, producing non-functional proteins. Its topical ophthalmic spectrum is broad among Gram-negatives, and importantly it covers Pseudomonas aeruginosa, the organism that causes the most rapidly destructive corneal ulcers, particularly in contact lens wearers.

It is used for external bacterial infections of the eye and its adnexa — conjunctivitis, blepharitis, keratitis — dosed every four hours in mild infection and hourly in severe cases until improvement. It is often the Gram-negative half of a fortified two-drop regimen for bacterial keratitis, paired with a cephalosporin for Gram-positive cover. Local adverse effects are mild: transient stinging, lid itching and swelling. Prolonged use causes punctate epithelial toxicity and encourages overgrowth of resistant organisms and fungi, so courses are kept short.

Sourcing notes. Off patent, inexpensive and widely registered, supplied as both drops and ointment, which are separate registrations. Combination products with dexamethasone carry substantial volume and are separate again. For fortified preparations, hospitals usually compound from injection, which is a different product entirely.
