Sertaconazole nitrate is a topical imidazole‑antifungal used primarily to treat superficial fungal skin infections (for example interdigital athlete’s foot) by inhibiting the fungal cytochrome P450 enzyme 14‑α‑demethylase, thereby blocking ergosterol synthesis and disrupting fungal cell membranes. Topical systemic absorption is minimal when applied to intact skin.

Sertaconazole is an imidazole antifungal with an unusual structural feature: a benzothiophene ring that resembles tryptophan closely enough to be incorporated into the fungal membrane, where it forms pores directly. So it works two ways — the usual azole inhibition of ergosterol synthesis, which is fungistatic, plus direct membrane disruption, which is fungicidal. It also has anti-inflammatory and anti-itch activity independent of its antifungal effect.

It is used for tinea pedis, tinea cruris, tinea corporis and cutaneous candidiasis, applied twice daily. The anti-itch effect is its practical differentiator: itching is what patients actually complain about, and relief in the first days improves adherence to a course that must run for several weeks after symptoms settle. It persists in the stratum corneum for around two days after application, which provides some margin for missed doses. Local burning and contact dermatitis are uncommon.
